Parent/Guardian Declaration

I confirm that as Parent/Guardian of the applicant, I have authority to act on his/her behalf. I hereby give permission for him/her to play the nominated sport. My child will abide by the rules and constituatio of Gibraltar Hockey. He/she is not a member of any other club or affiliated association involving the nominated sport. I further agree to releaseGibraltar Hockey and any of its office bearers, agents or representatives from any and all liability for any loss or damage, including consequential loss, which may arise in respect of, or incidental to its operations, including without limiting the foregoing, any liability arising from any negligent act or omission of the GIbraltar Hockey or its representatives, or any personal or property injury caused by the participation in any event organised by the club or its representatives or whilst using the facilities of Gibraltar Hockey.
